Forms for Cadet Applications

The following forms are used in the Cadet Applocation process. Each form can be accessed via the link to the NSCC National Website (HOMEPORT). Forms on the HOMPORT website are listed in form number order. We you see the form number and form name you are interested in, access either the PDF or Word format for downloading and printing for preparation of the lstest version of the form desired. No one will be denied admission to the Navy Sea Cadet Corps (NSCC) due to a medical disability. A cadet applicant whose medical condition precludes full, unlimited participation, should complete a Request for Accommodation (NSCADM 015) presented by their parent or guardian for review so the cadet may participate in NSCC activities to the maximum extent possible.

A medical examination similar to a high school sports physical is required for all cadet applicants to the Naval Sea Cadet Corps (NSCC) or Navy League Cadet Corps (NLCC). No one will be denied admission to the NSCC/NLCC due to a medical disability. Where a medical condition precludes full, unlimited participation, a Request for Accommodation (NSCADM 015) may be presented by the parent or guardian for review so the cadet may participate in NSCC activities to the maximum extent possible.

Adult applicants must be in good health commensurate with their age group and be free from any ailment or condition that would prevent them from satisfactorily performing their primary duty of supervising youth. NOTE: Adults not physically qualified to perform all duties may still participate with a waiver from NHQ considering the parameters of their expected contribution to the NSCC program.

NSCADM015 Request for Accomodation:

This form is used by perspective cadets with disabilities to apply for accomodation under the Americans with Disabilities Act. Refer to NSCC Medical Reference, Policy and Procedures Manual for guidance. To obtain this form click here.

NSCADM020 Report of Medical History:

This form is used to record personal medical history on NSCC/NLCC Cadets, Officers, Instructors and Midshipmen. To obtain this form click here.

NSCADM021 Report of Medical Examination:

This for is used to record personal medical examination on NSCC/NLCC Cadets, Officers, Instructors and Midshipmen. For cadet applications, please use this form when taking a physical examination that is similar to a high school sports examination. NSCADM004 Parental Agreement:

The form is used to solicit assistance from Cadet parents during the Cadet application process. To obtain this form click here.

NSCADM014 Scholarship Application:

This form is used to apply for NSCC Scholarships announced in annual Information Letters and referred to in NSCC Regulations Chapter 16. Do not use this form for NLUS Scholarship applications. To obtain this form click here.
